This is a savings plan, not a dental insurance plan. This plan is only honored at Salmon Creek Dental Center and cannot be used in conjunction with any other dental plan. All discounted rates cannot be combined with other promotions being offered at Salmon Creek Dental Center. Membership is not for re-sale. All Plan Premiums are Non-Refundable.
Premiums must be paid in full prior to receiving benefits.
Annual premiums are due January 1st of each benefit year that you wish to participate in.
Plan discounts are valid for the current calendar year only and expire after December 31st of the benefit year.
There will be a finance charge associated for services provided using Care Credit while covered under this plan. Patient portions are due in full on the day of services.
